• Computer Consent Form

     

    Certain Adult Education Students of the Plainview-Old Bethpage Central School District require network/system access to complete their course work. These students are provided access to network, email, and internet access. This access is subject to compliance with District policy.

     

    In order to utilize the District’s computer resources and have access to the Internet it will be necessary for you to complete and sign the Staff Network Use and Consent Agreement. By submitting this agreement, you are acknowledging that you have read all sections of the form and attachments and that you agree to adhere to the requirements set forth in these documents. If you do not have a submitted agreement on file, your access rights will be suspended.
